We offer initial outpatient consultations at the Inova Vascular – Fairfax outpatient office. To schedule an appointment, call 571-472-2327. Surgeries are performed at Inova Fairfax Hospital. 

We also provide virtual telemedicine options for new and established patients who are not in Virginia during their visits. New patients choosing telemedicine will need to provide existing imaging for review during the session. For more information, contact us at 571-472-2327.

Understanding aortic diseases 

The aorta, the largest artery in the body, carries oxygen-rich blood from the heart to various organs and tissues. Aortic aneurysms occur when the aorta's walls weaken or bulge, distorting its shape. The aorta can also tear, which is called an aortic dissection. These conditions can be life-threatening and may be detected through imaging tests. If untreated, aortic aneurysms and dissections can lead to serious, life-threatening complications.

  • Abdominal aortic aneurysm (AAA): occurs in the section of the aorta in the abdomen
  • Thoracic aortic aneurysm (TAA): Develops in the section of the aorta that runs through the chest and is often linked to genetic conditions
  • The following types of aortic disease require immediate emergency care:
    • Aortic dissection: a tear in the aorta's wall that causes blood to leak between its layers
    • Aortic rupture: a life-threatening tear resulting in blood leakage
  • Age (65+), especially in men
  • Smoking • Atherosclerosis (plaque buildup)
  • High blood pressure (hypertension) and family history
  • Genetic conditions (e.g., Marfan syndrome)

Inova's surgical treatment for complex aortic diseases 

We specialize in open and endovascular surgical procedures to manage large or complicated aneurysms and prevent serious issues like dissection or rupture. While we can treat urgent problems, our primary goal is prevention.

Our skilled surgeons perform open surgery to remove damaged portions of the aorta and replace them with synthetic grafts. This technique is used for severe cases, such as ruptures or dissections, requiring a chest or abdominal incision.

We offer minimally invasive endovascular techniques for patients who may not qualify for open surgery. Our surgeons insert stent grafts through small incisions in the groin or arm to strengthen the aorta and ensure blood flow. This method allows for shorter recovery times and includes: 

  • Endovascular aneurysm repair (EVAR): specifically for abdominal aneurysms
  • FEVAR: tailored to treat previously inoperable aneurysms in the upper abdominal aorta, ensuring better blood flow to side branch arteries
  • Thoracic endovascular aneurysm repair (TEVAR): focused on thoracic aneurysms

We offer hybrid aortic surgery, which combines open and endovascular approaches to provide the most effective repair strategy for a patient’s anatomy and any existing medical conditions. This innovative approach often involves multistep procedures but can reduce risks associated with open-chest surgery – particularly for patients with a history of smoking or COPD. Overall, our hybrid aortic surgery significantly enhances patient safety and outcomes.

Dr. Endicott focuses on high-risk patients, particularly those who are not candidates for open procedures, and is adept at multiple endovascular techniques.

Dr. Ryan offers expertise in intricate open and endovascular surgical repair within the ascending aorta and across the aortic arch as well as in the descending thoracic aorta.

David Spinosa, MD

Interventional Radiologist

Dr. Spinosa specializes in high-definition, real-time, intraoperative imaging during complex surgical challenges. He is also skilled in using microcatheters and other specialized tools to reach and repair small, delicate areas or connect grafts, particularly in the various branches of the aorta.
