What to expect as a family member
Our team is dedicated to providing comprehensive care to help individuals and their families reach meaningful goals throughout the rehabilitation process. We welcome and encourage family members to be an active member of their loved one’s stay.
The day your loved one arrives, our rehabilitation nurses and physician will assess your loved one. Therapy will begin the second day of your loved one’s stay. At this time, we will complete initial evaluations and goals will be established in collaboration with you and your loved one. A case manager or social worker will also meet with you during your loved one’s stay to guide you through the discharge process. Each evening, your loved one will receive a therapy schedule for the next day.
Discharge planning
Length of stay is determined by your loved one’s individual medical needs, progress in therapy and goals of care, but discharge planning begins on day one. This team approach to discharge planning helps to ensure families are supported and prepared as you plan for your loved one’s next level of care.
Prior to your loved one returning home, therapists and nurses complete family training sessions to review key information about your loved one’s care, make recommendations for home equipment needs and more. This valuable information is compiled into a notebook or binder for your family to keep and refer to at home. Your loved one’s personalized care binder will serve as a resource to help your loved one continue to make progress after your loved one is discharged from inpatient rehabilitation. Our team will work closely with you to help coordinate your loved one’s next level of care prior to discharge day.
What to bring
We recommend your loved one brings comfortable, loose exercise clothing including shirts, pants and sneakers. Four to five outfits that allow full mobility are preferred. Your loved one is welcome to bring toiletries and grooming items from home. Glasses and hearing aids are encouraged if needed.
